Time requirements for supporting medical reports:

1. The Application Form (Form 1) with two medical reports must be received and accepted by the Secretariat within 14 days of the last examination of the second doctor.
2. Applicant must ensure that they have seen the Subject within 14 days of the application date as required by law and confirm it in the application form. Otherwise, the application will be invalid even though the supporting medical reports complied with the following dates.
3. At least one examination must be carried out by an Approved doctor.
